Health & Well-Being

El Concilio California’s Health & Well-Being Department helps families access healthcare, prevent chronic disease, and build healthier lifestyles across the Central Valley.

We focus on removing barriers for insurance enrollment, transportation, medical appointments, and education about nutrition and prevention so families can grow, thrive, and stay well.

Access to Care and Preventive Support

When families can secure coverage, access preventive education, and attend medical appointments consistently, overall health improves and financial strain decreases.

Health & Well-Being at El Concilio California focuses on practical, community-based services that strengthen family stability and promote lasting wellness.

Participants report improved grocery choices, stronger portion awareness, and greater understanding of chronic disease prevention.

Our Community Wellness programs provide culturally responsive classes, outreach, and intervention focused on:

Medical Transportation Services (Stockton Area)

The Medical Transportation Services (MTS) Unit provides door-to-door, non-emergency transportation to healthcare appointments throughout Stockton.

Transportation is one of the most common reasons individuals delay medical care. Through MTS, clients who previously missed appointments are now receiving consistent treatment and preventive care.

“Since utilizing the service, I’ve been fully meeting my physician’s appointments and have seen an increase in my health and well-being.”
